At 21, Anamika should have been thriving—a university student studying abroad, exploring new places, and making memories. But for 14 months, even simple acts like chewing felt unbearable due to severe, persistent pain near her ears. Her jaw was constantly sore, her ears were buzzing, and the clicking every time she opened her mouth was a constant reminder that something was very wrong.

 

She had seen ENT specialists abroad, but her symptoms failed to resolve. Anamika wasn’t ready to accept that this was her only option.

When Pain Silences More Than Words

Anamika presented with a wide range of chronic and escalating symptoms, including pressure, buzzing, and ringing in her ears, as well as migraine-like headaches. She also experienced clicking on opening, soreness, and pain during mouth opening, along with pressure behind her eyes and persistent neck and shoulder stiffness. She reported daytime and nocturnal clenching and even signs of sleep-disordered breathing.

Clinical Findings:
  • Mouth opening: A comfortable 26 mm, with a maximum of 36 mm causing severe pain.
  • Muscle tenderness: Significant tenderness in the lateral pterygoid and temporalis muscles.
  • TMJ clicking: Noted on opening.
  • MRI: Right TMJ disc displacement without reduction and left condylar hypermobility.

Phase I: Stabilizing the Joint, Restoring Hope

We began with neuromuscular orthotic therapy—a custom-made lower full-coverage orthotic to offload the joints, reduce inflammation, and decompress the muscles. Due to her location, we monitored her progress with virtual follow-ups.

 

By September 2024, her mouth opening improved to 49 mm, completely pain-free. All her subjective symptoms (ear, head, neck, and muscle pain) resolved. The patient stated she felt “like a new person”.

Phase II: Orthodontic Alignment for Lasting Results

With her jaw now stable and pain-free, Anamika entered the next stage: orthodontics. This was done on a joint that had been healed and supported, with the goal of establishing long-term occlusal harmony. She completed her Invisalign aligner therapy in July 2025 with excellent compliance.

  • Mouth opening: The comfortable opening increased from 26 mm to over 45 mm, and the maximum opening went from 36 mm (with pain) to 49 mm (no pain).
  • Pain: The severe, daily pain was resolved.
  • TMJ sound: The clicking sound is now absent.
  • Quality of Life: Anamika’s quality of life, which was previously impaired, is now excellent.
